Liability Acknowledgment and Waiver

At Gem State Tennis Academy, our goal is to provide a safe, supportive, and fun environment for players to learn and grow through tennis. However, it’s important for parents and guardians to understand that tennis, like any physical activity, comes with certain risks. Please review the following information carefully to ensure you are fully informed about what to expect and the responsibilities associated with your child’s participation.

What You Should Know

Tennis Activities and Risks: Activities may include drills, matches, fitness exercises, and other tennis-related events. While safety is a top priority, risks such as falls, muscle strains, collisions, or equipment-related injuries can occur. Weather conditions (e.g., heat or rain) may also pose additional challenges.
Academy Commitment: Our coaches and staff are trained to prioritize player safety by using age-appropriate techniques, equipment, and supervision. However, no activity can be completely free of risk.

Your Responsibilities as a Parent/Guardian

Assumption of Risk: By enrolling your child, you acknowledge the inherent risks of tennis activities and accept these risks as part of their participation.
Health and Fitness Confirmation: You confirm that your child is in good health, with no medical conditions that might limit their ability to safely participate. If your child has any medical concerns, allergies, or limitations, you must inform Gem State Tennis Academy in writing before their first session.
Emergency Preparedness: In case of an emergency, you authorize Gem State Tennis Academy to seek appropriate medical care for your child. Any costs for treatment will be your responsibility.

What You’re Agreeing To

By signing this document:

You release and waive liability for Gem State Tennis Academy, its staff, coaches, and affiliates from any claims related to injuries, illnesses, property damage, or other incidents that may occur during participation.
You agree to indemnify and defend the academy against claims arising from your child’s participation, whether caused by negligence or unforeseen circumstances.
You understand that the academy may use its judgment to address emergencies promptly and in your child’s best interest.

Acknowledgment and Agreement

We are committed to making every session positive and rewarding for your child. By signing below, you indicate that you have read, understood, and agreed to the terms outlined above. You also confirm that you’ve provided accurate information about your child’s health and will update the academy of any changes as needed.

Media Release Consent

At Gem State Tennis Academy, we recognize the importance of capturing moments that showcase player development and progress. Photos and videos taken during lessons, matches, and events will be used solely for internal purposes, such as training, analysis, and academy documentation. Media will not be shared on social media or other public platforms unless explicitly approved by the parent or guardian.

Purpose of Media Usage

By signing this form, you consent to the use of photographs, videos, and/or audio recordings of your child for:

Internal coaching, training, and academy documentation.
Player progress tracking and analysis.
Academy presentations and internal reports.

Important Details

Privacy Considerations:

No media will be shared publicly on social media or other online platforms without direct parental approval.

Gem State Tennis Academy will not include your child’s full name, contact information, or personal identifiers in any media content unless additional permission is obtained.

All media will be handled respectfully to reflect the positive values of our academy.

Ownership of Media:

Media content captured by or on behalf of Gem State Tennis Academy is the sole property of the academy.

These materials may be used internally without the need for further consent or compensation.
